___ SET prior to service working with minors. (copy of official certificate certified by the Office of Save Environment); must be kept in file
___ Code of Ethics (Appendix III) signed.
___* Face to Face Interview (Appendix XIV) completed
___ Background clearance (Appendix V) completed, results received, reviewed and annotated prior to service with minors (copy to be kept on file)
___ Fingerprinting completed only if required by job description or for licensing.
___* Reference Checks– two received and the review annotated by reviewer.
___ Driver Information sheet (Appendix IX) completed if driving on behalf of the parish, mission, school or agency or if driving minors

___ Sign Code of Ethics again only if a major change in pertinent published policies.

COMMITMENT TO SAFE ENVIRONMENT
The Diocese of Gallup has put into place a process that assists in keeping our young and vulnerable people safe, by a thorough screening process. Appendix XII: Safe Environment Training Implementation
The screening process involves completing an application, interviews, signing a code of ethics and a background check which includes reference checks for all clergy, religious, seminarians, employees, volunteer leaders responsible for youth programs and those volunteers who work directly with minors or vulnerable persons. (The latter are otherwise known as “Affected Volunteers”).
All employees and affected volunteers who minister with minors must have or will have the juvenile sexual abuse training awareness program VIRTUS; virtus.org. This training requires a preliminary session-(phase one)- and then ongoing training-(phase two). See pp. 7-8 in the PPPM for further details.
